Breakdowns in communication remain one of the leading causes of preventable patient harm. Each year, more than 400,000 hospitalized patients experience avoidable adverse events, contributing to an estimated 100,000 deaths and more than $20 billion in associated costs. Research consistently shows that ineffective communication among healthcare professionals plays a central role. The Joint Commission has identified communication failures as a contributing factor in over 70% of sentinel events, and malpractice case reviews reveal that one-third of diagnostic errors involve at least one communication breakdown.
